The following forms are needed in order to register your child. You will need Adobe Reader to open the files, if you do not have it is available for free here.

The Physical form needs to be signed by your child's doctor and can be turned in at a later date

You will also need the following information:

• Photograph (head and shoulders only) approx 2" x 2" in size
• Birth Certificate
• Final report card form the 2007-2008 school year (provide when available)

The Flag and Jr. Pee-Wee levels are full. At this time we are accepting names for a waiting lists.

Registration fees are $150 per child, except at the flag level which is $100. There is also a family maximum registration fee of $400.

Note, there is also a mandatory pre-paid candy sale due at the time of registration of $100 for the first child, $50 for the second child, and no charge for any additional children after the second. If you do not wish to purchase the candy to sell, you can buyout for $50 for the first child and $25 for the second.
